MD 365 LLC (herein after “MSO”) is a management services organization for physician-owned primary care, and urgent care walk-in centers MD 365 Medical Care PLLC (“PLLC”)
PLLC has complete authority with regards to all medical decision-making and patient care. MSO shall, in no way, determine or set the methods, standards, or conduct of the practice of medicine or healthcare provided at, or by, or through PLLC, or by any of its professionals. MSO provides PLLC the license to use this website and content ownership. MSO also provides consultative services and offers recommendations to the PLLC to consider, reject, revise, and/or adopt as it deems fit.
This privacy notice applies solely to information collected by this website, conditions for use, information collection, and sharing. Any website forms should only be used for legitimate purposes.
The forms on this website must be used only for general correspondence. You must not submit any sensitive information, such as your Social Security number or medical history, through any of the forms on this website.
