MD365™ LLC (herein after “MSO”) is a management services organization for physician-owned -multispecialty medical practice Brookhaven Heart PLLC (“PLLC”)

All medical services are provided by the PLLC, which has complete authority with regards to all medical decision-making and patient care. MSO shall, in no way, determine or set the methods, standards, or conduct of the practice of medicine or healthcare provided at, or by, or through PLLC, or by any of its professionals. MSO provides PLLC the license to use this website and content ownership. MSO also provides consultative services and offers recommendations to the PLLC to consider, reject, revise, and/or adopt as it deems fit. 

Website Privacy

This privacy notice applies solely to information collected by this website, conditions for use, information collection, and sharing. Any website forms should only be used for legitimate purposes. 

The forms on this website must be used only for general correspondence. You must not submit any sensitive information, such as your Social Security number or medical history, through any of the forms on this website. 

You may use our secure patient portal to communicate with our office for your medical needs, such as medication refills, appointment requests, or reviewing your laboratory tests. 

We will not forward your communication with any third parties except as authorized or required by law. We will use the information you submitted to contact you, regarding the reason you contacted us. Only employees who need the information to perform a specific job are granted access to the information you submit

We take reasonable precautions to protect your information. You agree not to hold PLLC, or MSO or any of its staff for network or internet infractions beyond its control. Online communications may travel beyond geographic boundaries and is easier for a third party to intercept, forward or change it without prior knowledge. Employers, internet service providers, etc. have a right to inspect and keep online communications transmitted through their system.
