COVID-19 Vaccination Appointments at MD365
We are notifying our patients when you can schedule your vaccination. This is based on both your eligibility as determined by New York State and our available supply.
You will be notified by email or text message that you are able to schedule an appointment based on the contact information you provided when you registered with us. Please make sure your contact information is up to date. If you have not been to us before, you can schedule appointment by calling us at (631) 772 4646 or Book Online
We are scheduling vaccinations only after we have confirmed that we have doses available for distribution. This is to make sure we do not have to cancel vaccination appointments after they have been scheduled.
Vaccine Information and other Considerations
When we notify you that you are eligible to schedule your vaccination, we encourage you to schedule your appointment as soon as possible, as availability is limited.
When you are offered a vaccination appointment with us, you may receive any of the three vaccines that are currently approved—Pfizer–BioNTech, Moderna, and Johnson & Johnson/Janssen—based on our available supply. While you will not be able to choose the vaccine you receive, you will be informed which one you receive at the time of your appointment. The Pfizer–BioNTech, Moderna, and Johnson & Johnson vaccines have all been proven to be highly effective at preventing severe disease.
Most people can immediately schedule an appointment after you receive the email or text notification.
Please do not schedule a vaccination or come to MD365 to be vaccinated if any of the following statements apply to you:
- You received your first dose of the vaccine somewhere else. Please schedule your second dose at the same location you received your first dose.
- You are currently feeling sick.
- You are on isolation or quarantine during the past 10 days due to potential exposure to known COVID 19 patients.
- You have an active COVID-19 infection. You can schedule 10 days after you have received a positive COVID-19 test result or 72 hours after your fever ends.
- You received a COVID-19 diagnosis and were treated with COVID-19 monoclonal antibody infusions or convalescent plasma within the past 90 days.
- You have had any vaccine in the past 14 days.
Depending on the vaccine you receive, you may need one or two doses. If you require two doses, your second dose will be scheduled at the time of your visit for your first dose. You will be offered appointments between three and four weeks after your first dose, depending on the vaccine you received.
