Instructions for Immigration Physicals

1. Obtain the i-693 Form

Download the i-693 form from Alternatively, you can obtain a copy from any of our offices in Patchogue or New Hyde Park or request it to be e-mailed or mailed to you.

2. Complete the Form

Read the instructions carefully and complete your section of the form.

Do not sign and date Part 2; this section must be completed in the presence of the Civil Surgeon.

3. Bring Identification

Bring a government-issued photo identification, such as a passport or driver’s license.

4. Payment

Please bring a form of payment. Immigration Physical fees (including tests or vaccinations if needed) are not covered by your health insurance. We accept cash, checks, or credit cards.

5. Vaccination Records

Bring your vaccination records. If you do not have access to them or are unsure, we will perform necessary blood tests to determine the vaccinations you may need.

6. Immigration Physical Costs

Immigration Physical Fees: $200.00

Additional costs:

You may require blood tests and urine tests, which incur additional charges. The Civil Surgeon will explain the necessary tests and provide pricing details. Additional vaccinations, if needed, will also be charged separately.

7. Incomplete Form

After your first visit, you will receive an incomplete form. It is your responsibility to keep it safe and bring it back for your follow-up visit.

8. Follow-Up Visit

In most situations, a follow-up visit will be necessary to review the results of additional tests, provide any required vaccinations, and, in rare cases, make referrals if necessary. This follow-up visit is provided at no additional charge.

9. Completed Forms

Once all the required information is obtained, the Civil Surgeon will sign and date the completed forms and hand them over to you in a sealed envelope.
